Description of best practice: This program serves black elders, elder walking groups, English only. However any one who identifies as a black elder can participate. This program focuses on coalition building, outreach, networking, and community sustainability by using the data that has been collected.

During the planning phase of the project, Boston Public Health Commission (BPHC) developed a coalition of community based service providers, government agencies, citizen groups and elders who are concerned with the health of African American elders in Boston. Collectively, BPHC and the Coalition devised a Community Action Plan (CAP).

Boston's Alliance for Community Health is comprised of seventeen neighborhood coalitions working together to improve the overall health of their residents, with a focus on health promotion and prevention. Each neighborhood chooses a health topic to work on, and their projects include input from community residents, health care providers, and other key neighborhood stakeholders.

Activities include:

Mini-Grants
The Boston Public Health Commission, in partnership Massachusetts Department of Public Health, the Medical Foundation, the Massachusetts Prevention Center, and local hospitals, provides small grants to neighborhood groups to support health promotion activities.

Community Meetings
Each year, the Alliance organizes a series of community meetings in each of Boston neighborhoods. The community meetings are an opportunity for the Boston Public Health Commission to share neighborhood health data, get feedback from community residents, and maintain an open dialogue between the Commission's and the neighborhoods it serves.

Link Team
Staff from the Boston Public Health Commission, the Massachusetts Department of Public Health, and the Prevention Center provide support to community residents working to improve the health of their communities. Link Team Leaders provide neighborhood groups with health indicator data, funding opportunities, skills training, and assistance with their community health initiatives.
