Description of best practice: The case management model that we use is that of a community/hospital based so that our consumers get access to health care and be connected to services in the community where they reside. In addition, clients receive follow up care by the same case manager. Haitians and their families infected and affected by HIV. Case managers primarily at Boston Medical Center speak Haitian Creole and French.

We deal with a stigmatized population, the native of work (HIV/AIDS) consumers are more comfortable dealing with one person in the hospitals and in the community.

This is funded by the state and city of Boston.
