Description of best practice: Volunteer AmeriCorp members – nurses, nurse practioners, and pre-med students provide outreach, screening and health promotion in community based sites. We link uninsured to community resources such as the Dr. Elias Health Center for Adults. We serve elderly housing, soup kitchen, malls, homeless shelter and churches and predominantly in English, but focus on Portuguese as well.

This practice is successful because we provide radio outreach (high literacy rate). This program is based on the AmeriCorps model. We use assessments to determine community needs.

We are funded through different grants such as AmeriCorps, MA Service Alliance, BC/BS Foundation of MA and Harvard Pilgrim Foundation.
